AGING WELL - A comprehensive community-based program of healthy aging for men and women 50 and better. Emphasizing wellness and prevention, Aging Well empowers older adults to stay active and engaged in community life and to remain healthy, safe and independent at home for as long as possible. COMMUNITY HEALTH CENTER (Moffat County Only) - Provides a primary care medical home, case management and referrals services, assistance with prescriptions, behavioral health counseling and brief interventions for a variety of mental health and substance abuse related issues.
